Ballot form

A piece of paper you fill in stating how you wish to vote for a particular award. The ballot form should say on it where to take it once you've filled it in. At an Eastercon, you will probably receive a ballot form for the Doc Weir award and possibly also the Phlosque award, as well as several hall costume tokens.
